Perianal Fissures help for dog

My German Shepherd has perianal fissures and I am wondering if there are any alternatives to surgery and protopic. The meds aren’t working and surgery seems to not help.

    Tough problem. Surgery is risky if the fistula is extensive. Long term antibiotics, atopica, immuran, steroids, irrigating and cleansing the rear end. Tail amputation is possible if rectal hygiene is a problem.

    What medications are you administering? The current standard of practice for medical management is Neoral 5 mg/kg by mouth every 12 hours with blood tests performed at a specified interval to determine if a dosage adjustment is indicated, ketoconazole 10 mg/kg by mouth every 12 hours in conjunction with a hypoallergenic diet (e.g.) prescription Z/d diet, as it is becoming more evident that these fistulas represent intestinal food hypersensitivities. There is also a topical potent anti-inflammatory called tacrolimus.
